835 PLB provider-level adjustments: WO, L6, FB and more

The PLB segment adjusts the whole payment, not a specific claim. It's why a payer's deposit can be smaller (or larger) than the claims it paid.

The sign

On a PLB, a positive amount reduces the payment (money taken back) and a negative amount increases it (money added, such as interest). The rule is: sum of CLP04 claims paid − sum of PLB amounts = BPR02 payment.

Example

Claims paid $90.46. PLB WO (overpayment recovery) 25.00 and L6 (interest) −1.25. Payment = 90.46 − 25.00 + 1.25 = $66.71.

The reference after the reason (for example WO:20260901ABC) usually identifies the claim or letter the recoupment relates to. Remitgrid lists every PLB under its payment and in the Excel workbook's Provider adjustments sheet.
